Rules Reference Sheets

Not sure if this will be of any use to you, but I actually make my own for every army list. It is not a full reference of the game phases though, just all the information I need for each unit so at most all I would ever need is the rulebook and the relevant document.

I'll give you a quick example as I'm sure it would be easy to follow my template if you wanted to replicate it:

p.1 - Army List: The first page is my army list. This just shows what units I am using, what additional equipment I have purchased and their total points cost.

p.2-3 - Formation Bonuses / Rites of War; 'Common Special Rules'; Warlord Traits: This section summarises all the key rules that are faction wide, so that I don't have to write them out in every unit entry. This includes the effects of a Formation or Rite of War as well as what any Special Rules that are gained do (i.e formation grants Crusader, this is then listed as a bonus of the formation and the rule is summarised so that I not only know what it gives my units, but what it does). 'Common Special Rules' refers to all faction wide rules (i.e. Space Marine Chapter Tactics, Chaos 'Daemon of...' rules and Legiones Astartes...) - rules that are so frequently used they are summarised right at the front to save searching in every unit entry. Again, what each rule does is summarised from the relevant codex or the Special Rules section of the rulebook. Warlord Traits are also listed here.

p. 4...: Unit Profiles: The full profile for every unit from the army list is then listed. This includes full characteristics profiles, a list of equipment with the accompanying profile for every item/weapon and a list of special rules, each of which are summarised so I immediately know what they do (unlike in the Codexes which just state the name of the rule but not what it does). Permanent stat changes are included in the profile too, to make it very clear (i.e a unit with Mark of Slaanesh gains +1 Initiative and so their profile would be written as I5 not 4 with +1 in the special rules or the profile for a Monstrous Creatures close combat weapon is set with AP2 regardless of the base stat of the weapon due to the effects of Smash). Space is included on the profiles of Psykers to write in the powers they generate.

Appendix: Psychic Powers - All the Psychic Disciplines I will be using are listed here as well as a breakdown of each power.

I find this SO useful as it effectively means I no longer need to carry the army codex / red books for 30k and I have all the special rules, weapons and psychic powers listed in the unit entries to save looking them up every time. This essentially means I only need the main rulebook for universal things like the game turns, vehicle rules and terrain etc. as everything specific to my army is on the sheet.

Can anyone recommend reliable rules reference sheets for both HH and 40K?

Over on Dakka, there used to be a fellow who created a 40k reference table. Looks like it was last updated in June but it can help give you a quick sheet for the basic rules:
